- This is an editable form so please fill out as completely as possible. If you need assistance please contact our Financial Secretary, or Grand Knight. (It is important that the applicant check the practicing catholic box and enter the parish information.) Former council and location information is very desirable for transfers)
- The applicant must print and ink sign the Form 100; Supreme Council will not accept an electronic signature at this point.
- The applicant, after signing, can scan and email the form to the Financial Secretary or Grand Knight, if they prefer. Also, it is very important to make sure the form is legible; printing, scanning and emailing all degrade the quality.
- Optionally, the candidate can mail or hand deliver the form originals to the Financial Secretary or Grand Knight or simply drop it off at the church office in an envelope addressed to Knights of Columbus.
